Membership Applications
 

IMPORTANT APPLICATION INFORMATION:

• Per the Commonwealth of Virginia, the Company is required to conduct a criminal history check on any applicant requesting membership.

• Per Company By-Laws, all applicants for Junior Membership must provide a parental or guardian signature and enclose a copy of their most recent report card.

• Per Company By-Laws, junior members are not permitted to be a member at another fire/rescue company while a member with Middletown. 

• Please enclose your membership dues of $1.00 with your application.

• You may only use one (1) Company member as a reference.  We prefer NO family members be used as references.

• Please provide copies of any certifications that you currently hold (CPR, EMT, FFI, etc.)
   

• We no longer accept any new applications in the month of December.  If you would like to submit your application in December, it will be held and voted on at our February Company meeting. 

MEMBERSHIP INTERVIEWS

All applicants are required by the Company, to meet with the Membership Committee for a brief interview.  The interviews are informal and tend to last approximately 20 minutes.  Each applicant must be interviewed, prior to your application being brought before the Company for vote.

Once your application has been received, you will be notified by email (or letter), that it has been received by the Committee and will confirm your interview time slot.

We must have ALL references and your dues, prior to your interview. 
Junior applicants need to submit their latest report card with their application.
